The Continuum Hospice Foundation
The Continuum Hospice Foundation is an independent 501(c) (3) charitable organization that is dedicated to raising the funds necessary to help hospice and palliative care patients. The Foundation is run entirely by a team of caring volunteers and knowledgeable staff who raise funds and build community awareness in support of terminally ill patients and their families.
The Continuum Hospice Foundation provides funding for the extraordinary needs of our patients and families that extend beyond the borders of the traditional hospice benefit. Funding interventions can range from fulfilling a final wish for a quiet anniversary celebration, to helping pay for utility bills or covering the travel costs to bring a distant family member to a patient’s bedside. Specific programs include our Make a Wish program, special bereavement programs and educational programs in the communities we serve.

Sometimes, patients’ wishes are complicated. They may dream of going on a long-delayed honeymoon, attending a family reunion out of state or seeing a final game by a favorite sports team. The Continuum Hospice Foundation works collaboratively with the clinical teams at Continuum Care Hospice and Affinity Health Management to carry out our patient’s last wishes, thereby creating lasting memories for patients and their loved ones and adding meaning to their days.
Whether a patient needs an air conditioner to rest comfortably at home, or help paying rent or grocery bills, The Continuum Hospice Foundation is ready and willing to assist with life essentials in any way possible.
